Thanks so much for your interest in serving with minors at First Baptist Church Arlington.  We take the safety of our preschoolers, children and youth very seriously.

You will need to download all three of the documents below.  Please fill them out completely and return in a sealed envelope to Terry Bertrand in the Church Offices.  You can also mail them to:
